Referrals should be submitted by social workers, case managers, school staff, or community professionals supporting the youth.

Use this form if you are a social worker, school staff, case manager, or community partner referring a young adult ages 18–24 who is aging out of foster care or experiencing housing instability in New Jersey.

Once we receive the referral, our team will review the information and follow up with you for next steps.

Eligibility for Referral

We accept referrals for young adults who meet the following criteria:

• Ages 18–21 transitioning out of foster care

• Ages 22–24 experiencing housing instability (Straight Path to Success program)

• Located in New Jersey

• Able to participate in a structured, supportive program

• Not in immediate crisis or requiring emergency placement
